Composition Each gram of ointment contains: • Betamethasone (as Betamethasone Valerate) — equivalent to 0.1% w/w • Neomycin Sulphate — equivalent to 0.5% w/w • Ointment base — q.s. Pharmacological Class • Betamethasone: Topical Corticosteroid • Neomycin: Aminoglycoside Antibiotic

Indications

Cekovate-N is indicated for the treatment of corticosteroid-responsive dermatoses with secondary bacterial infection, including:

  • Infected eczema
  • Infected dermatitis
  • Prurugi nodularis, psoriasis, lichen simplex chronicus, lichens planus, seborrhoroiec dermatitis, contact dermatitis, anal or genital intertrigo, Onitis externa , insect bites , prickly heat.
  • Atopic and contact dermatitis with bacterial contamination and involment
  • Inflammatory skin conditions complicated by susceptible organisms

Mechanism of Action

  • Betamethasone: Acts by suppressing inflammatory responses and inhibiting the release of inflammatory mediators, thereby reducing redness, swelling, and itching, erythema , edema and pruritus.
  • Neomycin: Inhibits bacterial protein synthesis by binding to the 30S ribosomal subunit, resulting in bactericidal activity against susceptible organisms

Dosage and Administration

  • Apply a thin layer to the affected area 1–2 times daily or as directed by a healthcare professional. The number of times you use your medication may be reduce by doctor as the skin gets better or the doctor will shift you towards another steroids that does not contain neomycin. Do not use for more than 7 days.
  • If you are using it with emollient (moisturizer) preparation aloow Cekovate N ointment to completely absorb before applying emollient.
  • Use for short-term treatment only
  • Avoid occlusive dressings unless advised by a physician

Pediatric and elderly patients:
Use with caution; prolonged use should be avoided.

Warnings and Precautions

  • Prolonged or excessive use may cause skin atrophy, striae, or systemic corticosteroid absorption
  • Neomycin may cause contact sensitization, especially with extended use
  • Avoid use on large body surface areas or broken skin
  • Use with caution in pediatric patients due to increased risk of systemic absorption
  • Discontinue if irritation, sensitization, or superinfection occurs
  • If the infection persist for more than 7 days of use speak to your doctor.
  • Some medications affects how cekovate-N works or you will likely to have side effects with for example Ritronavir, itroconazole , neuromuscular blocking agents, other medications containing neomycin

Contraindications

Cekovate-N Ointment is contraindicated in patients with:

  • Hypersensitivity to betamethasone, neomycin, or other aminoglycosides
  • Viral skin infections (e.g., herpes simplex, varicella , cold sore , chicken pox)
  • Fungal skin infections
  • Tubercular skin lesions
  • Acne vulgaris, rosacea, or perioral dermatitis
  • Ophthalmic use or application in the ear with a perforated tympanic membrane
  • Children under 2 years.
  • Do not use on rashes around mouth, itchy skin which is not inflamed, inflamed flaky skin of outer ear or ear canal if your ear drum is perforated, itching around anus or genitals

Adverse Reactions

Common

Burning or itching at application site, Skin irritation, Dryness.

If your skin become swollen durinfg use you are allergic to Cekovate-N

Less Common

Skin thinning, Hypopigmentation, Folliculitis

Rare but Serious

Allergic contact dermatitis, Secondary infections, Suppression of hypothalamic-pituitary-adrenal (HPA) axis (with prolonged use)

Very rare side effects

It may affect upto 1 in 10,000 people , If use prolong period of time or airtight dressing it may cause Obesity , moon face / Rounding of the face Skin thinning ( this may cause srtech marks) , skin wrinkling , dryness , appearance of the blood vessels under your skin, skin color changes, body haid, hair loss/ damaged hair/ lack of hair growth.

If you have psoriasis you may get raised bumps with pus under the skin and its known as postular psoriasis

Following side effects may also occur in children

Delayed weight gain , slow growth, Very rare side effects shows up in blood test or when doctor gives you examination

Incease level of cortisol in your blood , Increase level of sugarin your blood or urine, High BP, Cloudy lens in the eye (Cataract), increase pressure in the eye(glaucoma),  weakening of the bones through gradual loss of minerals (osteoporosis).

Seek medical help if you have any of these or if you notice any other side effects which is not listed here

 

Management of Adverse Reaction

  • Mild local reactions generally resolve after discontinuation
  • Stop treatment immediately if hypersensitivity or worsening infection occurs
  • Avoid prolonged continuous use
  • Initiate appropriate alternative therapy if fungal or viral infection is suspected

Use in Pregnancy and Lactation

  • Use only if the potential benefit justifies the potential risk
  • Avoid prolonged use, large surface application, and occlusive dressings
  • Use under medical supervision

Storage

Store below 30°C.
Protect from light.
Keep out of reach of children.
