Indications

Vitamin D3 injections are indicated for:

  • Treatment and prevention of Vitamin D deficiency or insufficiency
  • Management of rickets and osteomalacia
  • Support in hypocalcemia due to Vitamin D deficiency
  • In patients who cannot take oral Vitamin D or have malabsorption disorders

Dosage and Administration

Adults:

  • Typical dose: 6000–60,000 IU intramuscularly (IM) or subcutaneously (SC), frequency as prescribed by physician

Children:

  • Dose varies based on age and severity of deficiency (usually 600–2000 IU/kg over a period, as prescribed)

Administration Notes:

  • Administer deep IM injection or subcutaneous injection as recommended
  • Follow physician instructions regarding frequency and dose
  • Avoid self-administration without guidance

Mechanism of Action (MOA)

  • Vitamin D3 is converted in the liver to 25-hydroxycholecalciferol and then in the kidney to 1,25-dihydroxycholecalciferol (calcitriol)
  • Promotes intestinal absorption of calcium and phosphate
  • Supports bone mineralization and skeletal health
  • Regulates serum calcium and phosphate levels

Contraindications

  • Known hypersensitivity to cholecalciferol or any component of the injection
  • Hypercalcemia or hypervitaminosis D
  • Severe renal impairment with hypercalcemia
  • Patients with sarcoidosis or other granulomatous diseases causing hypercalcemia

Precautions

  • Monitor serum calcium and phosphate levels during therapy
  • Use with caution in renal or hepatic impairment
  • Avoid excessive sunlight or other vitamin D supplements during therapy to prevent toxicity
  • Inform your doctor if pregnant or breastfeeding

Adverse Reactions / Side Effects

Common:

  • Pain or swelling at injection site
  • Mild nausea or constipation

Rare but serious:

  • Hypercalcemia: fatigue, confusion, muscle weakness, excessive thirst or urination
  • Allergic reactions: rash, itching, swelling, difficulty breathing

Handling Side Effects:

  • Mild injection site discomfort: apply cold compress or rotate injection sites
  • Signs of hypercalcemia: notify healthcare provider immediately

Storage

  • Store at 2–8°C (refrigerated)
  • Protect from light
  • Keep out of reach of children
